Client upload the files:
After a patient visit to a physician, a patient chart is prepared and other clinical information is scanned.
It contains the brief details of the diagnosis and procedures performed.
The Client will upload these information to their secured FTP server.
Downloading & allocation of files:
The files uploaded from the Client using the FTP site, will be downloaded by Medical Coding Supervisor and allocated to the appropriate Pre-Coders and Coders.
Pre-Coding:
The Pre-Coder will enter details such as Place of Service, Physician Name, Hospital Name, Document Id, etc.
Coding:
The Coder will read the patient chart, match the problem to its corresponding numerical ICD-9 code and assign the proper diagnosis code with its numerical CPT code. The Coder checks the compatibility of the diagnosis with the procedure code. These two codes and any modifier codes describes the patient's problem and its treatment.
Verification:
The Experienced or Specialty coders will verify the coding work and changes will be made if necessary, to achieve the quality levels of 95%-98%.
Quality Audit:
The quality audit checks the complete pre-coding and coding with available references to ascertain compliance with coding guidelines and NCCI edits.
It is done to give a 100% job accuracy.
Upload files to the Client:
The Medical Coding Supervisor generates the final output sheet of coded records and uploads the coded records in client specified format, i.e. Excel, CSV, TXT, XML to their secured FTP server.
